Group Project Instructions & Forms

All students participating in a GROUP Project must complete FASF registration forms AND forms required by ISEF.

Note:  All group projects will be judged against each other regardless of grade level.

Once you have selected a topic in an area of science in which you have some particular interest, experience or resources available to you, you may begin registration for FASF.  The following is a guide.  You can also email director@flintsciencefair.org to ask questions.

Read the FASF Rules & Regulations and then use ISEF Rules Wizard to review the Intel International Science & Engineering Fair (ISEF) rules that apply to your project area.  FASF guidelines for projects generally follow those of Intel ISEF.  There are some rules specific to FASF, however, which you should read first.  Print any forms you will need from these sites.  Some project areas need more forms than others.

Go to the ISEF Rules Wizard at http://apps.societyforscience.org/isef/students/wizard/index.asp before starting your research, to see which forms and approvals will be required. The forms are for ISEF Finalists, but we also use them for FASF.

Note: Each team member must submit an individual signed Approval Form (1B).

Team Members can:

Team members must jointly submit all forms in a single combined package.

  • Registration Forms for each student
  • One Required Safety Checklist signed by Adult Sponsor
  • One Abstract
  • One Team Research Plan
  • Approval Forms (1B)—one for each student

If your experiment involves any of the following categories—follow the directions below.  From the following link scroll to the needed form and print page(s) as needed. 2010 ISEF Forms.

  • Humans - complete ISEF Form 4: Human Subjects and Informed Consent Form. Remember you need a consent form completed for every human subject!
  • Vertebrate animals – complete ISEF Form 5: Vertebrate Animal Form (5A and 5B)
  • Potentially hazardous Biological Agents – complete ISEF Form 6A: Potentially Hazardous Biological Agents Form or Form 6B: Human and Vertebrate Animal Tissue Form
  • Hazardous Chemicals, Activities and Devices - complete ISEF Form 3 Risk Assessment Form.
  • None of the above (No additional forms required)
