Elementary Division Instructions & Forms
Elementary Division (Grades 4-5) All students in grades 4-5 must complete FRSF registration forms.
Once you have selected a topic in an area of science in which you have some particular interest, experience or resources available to you, you may begin registration for FRSF. The following is a guide. You can also email director@flintsciencefair.org to ask questions.
Elementary Division projects follow simpler rules than those for the senior division. All elementary division projects will be judged as one category.
- Complete your FRSF Application on-line or print the FRSF forms from the web site or use the paper forms from pages 13-16 of the Handbook and mail them to the address below.
- Keep copies for your files.
- The Required Safety Checklist and the Approval Form 1B forms must be signed by a parent or adult sponsor and mailed to the address below or can be brought to the registration table on Setup March 16, 2010.

If your experiment involves any of the following categories—follow the directions below. From the following link scroll to the needed form and print page(s) as needed. 2012 ISEF Forms.- Humans - complete ISEF Form 4: Human Subjects and Informed Consent Form. Remember you need a consent form completed for every human subject!
- Vertebrate animals – complete ISEF Form 5: Vertebrate Animal Form (5A and 5B)
- Potentially hazardous Biological Agents – complete ISEF Form 6A: Potentially Hazardous Biological Agents Form or Form 6B: Human and Vertebrate Animal Tissue Form
- Hazardous Chemicals, Activities and Devices - complete ISEF Form 3 Risk Assessment Form.
- None of the above (No additional forms required)

If you know that your project involves human subjects, vertebrates or potentially hazardous biological agents (cultured microorganisms, R-DNA, fresh tissue, blood or body fluids, parasites) you will find sections of the rules that apply exclusively to your project. A Review Committee must approve your research design before you begin your work.
